Documents, one by one
- W-9The IRS form with your business name and tax ID.
- Business registrationProof your business is registered and active with your state.
- Local business licenseYour city or county business license or tax receipt, if your area requires one.
- Your insurance certificateThe one-page certificate your insurance agent sends, with Thoro Clean added.
- Workers' compA workers' comp policy, or your state's exemption, if your state allows one.
- Equipment checkA photo of your water reclaim / wastewater recovery setup, when we ask.
